Application is hereby made to Altitude Construction Supply for a commercial credit account. The undersigned represents and warrants that credit extended hereunder will be utilized solely for business or commercial purposes and not for personal, family, or household purposes. Applicant represents that applicant is solvent and that any information furnished by applicant in connection with this application is accurate and complete and acknowledges that Altitude Construction Supply can rely thereon. Altitude Construction Supply agrees that this information will be held in the strictest confidence and will be used for credit purposes only.

Applicant agrees that payment terms are Net 30 days from invoice date and that a monthly Finance Charge of 1% (12% APR) will be assessed and payable on all sums 31 days or older, as of the last regular working day of each month. In the event of default and in addition to all other available remedies, finance charge on any past due balance will be calculated from the date of default.

All sales are subject to Altitude Construction Supply standard terms and conditions.
